What can I expect at an appointment?
Every session, you will work one-on-one with a licensed Doctor of Physical Therapy. Treatments may include individualized exercises and activities, movement training, or localized manual therapy techniques based on your specific needs. Initial evaluations for physical therapy and performance assessment are 60-75 minutes. Follow-up sessions are 60 minutes.
How can I prepare for a session?
Be sure to fill out the required paperwork located in the patient portal prior to your initial session. Ascent Physical Therapy and Performance will send these to you electronically prior to your appointment.
When you arrive for a session, be sure you are ready to move and dressed in attire that you feel comfortable working out in.
How do I get to the clinic?
Ascent Physical Therapy and Performance is located within Acuo CrossFit. When you arrive, park in the available parking spaces across from the gym. Enter through the door on the far right end of the building. There is a small waiting room through that first door. Your physical therapist will meet you there when it is time for your session.
Do you take my insurance?
Ascent Physical Therapy and Performance is a cash-based physical therapy practice that operates out-of-network from traditional insurance companies. Payment will be collected at the time of service. Ascent will provide an invoice (or Superbill) which you can submit to your insurance company for partial reimbursement if you have out-of-network physical therapy benefits. You may also use HSA or FSA funds to pay for your physical therapy.
Why out-of-network?
Due to the current nature of insurance companies, physical therapy clinics are seeing decreasing reimbursement rates for the services they provide. This makes it increasingly difficulty to provide the adequate level of care to effectively treat each patient. Alternatively, the out-of-network model bypasses the normal reliance on the insurance industry, allowing your therapist to provide each client with the best individualized and evidence-based treatment in order to see lasting results. It allows you to focus on your needs and goals without being restricted by the limitations and timelines of insurance companies.
What are my out-of-network benefits?
Most insurance companies, with the exception of Medicare, Medicaid, and some HMO plans, offer out-of-network benefits. This means that your insurance company will provide reimbursement for services received out-of-network from a physical therapist who is not a participating provider with your insurance company.

Can I use HSA or FSA funds?
Yes! This is a great reason to use your Health Savings Account (HSA) or Flexible Spending Account (FSA) in order to see the provider you want to see.
